In-Home Care Assessment

Inviting an in-home care provider into your life is a big step, especially when you’re searching for trusted in-home care in Baton Rouge, Louisiana. It’s normal to feel unsure about what the process looks like or what will be asked during the first visit. At Moore Care, a locally owned and operated home care provider serving Baton Rouge and the surrounding areas, we believe the in-home care assessment should feel supportive, informative, and reassuring, not overwhelming.

Here’s what you can expect during your first in-home care assessment and how it helps us create a care plan that truly fits your needs.

What Is an In-Home Care Assessment?

An in-home care assessment is a one-on-one meeting conducted before services begin. This visit allows us to understand your unique situation, determine the appropriate level of care, and design a personalized care plan.

At Moore Care, all assessments are completed by Sheri, our Director of Care, who proudly serves families throughout the Greater Baton Rouge Area. With years of experience in senior care, Sheri takes the time to listen carefully, observe thoughtfully, and ensure no detail is overlooked.

The assessment fee is $75, and the good news is that this fee is refunded on your first invoice once services begin.

Who Should Be Present?

We encourage clients, family members, and anyone involved in care decisions to be present if possible. Having loved ones there allows everyone to ask questions, share concerns, and feel confident moving forward together.

What Happens During the Assessment?

Every assessment is tailored to the individual, but most visits include the following components:

1. Conversation and Care Goals

Sheri will begin by getting to know you, your routines, preferences, concerns, and goals. This is your opportunity to share what’s working well, what’s becoming challenging, and what kind of support would be most helpful.

2. Health and Safety Review

The assessment includes a general overview of:

  • Medical conditions and diagnoses
  • Medications and mobility considerations
  • Fall risks and home safety concerns
  • Memory, cognitive, or behavioral changes

This helps us determine the safest and most effective approach to care.

3. Daily Living Needs

We’ll discuss how assistance may be needed with activities of daily living, such as:

  • Bathing and personal hygiene
  • Dressing and grooming
  • Meal preparation and nutrition
  • Light housekeeping
  • Transportation or companionship

The goal is to provide the right level of help while maintaining independence and dignity.

4. Personality, Preferences, and Routine

Great care goes beyond tasks. Sheri will take time to learn about personal preferences: likes, dislikes, routines, and personality traits, so we can match caregivers and services appropriately.

Creating a Personalized Care Plan

After the assessment, Sheri uses the information gathered to determine the appropriate care level and design a personalized care plan. This plan outlines recommended services, scheduling, and any special considerations to ensure care feels seamless and supportive.

Nothing is one-size-fits-all. Care plans are designed to evolve as needs change.

A Time for Questions and Peace of Mind

The in-home care assessment is also your chance to ask questions about services, caregivers, schedules, or what to expect next. Our goal is for families to leave the assessment feeling informed, confident, and at ease.

Why the Assessment Matters

This first visit sets the foundation for quality care. By taking the time to thoroughly assess needs upfront, we’re able to provide care that is safer, more personalized, and better aligned with each client’s lifestyle.

Ready to Take the Next Step With In-Home Care?

If you’re considering in-home care for yourself or a loved one, our assessment is the first step toward compassionate, personalized support. With the $75 assessment fee refunded on your first invoice, it’s a risk-free way to ensure you’re getting the right care from the start.

If you’re searching for reliable, compassionate in-home care in Baton Rouge, Louisiana, Moore Care is here to help. Contact Moore Care today to schedule your in-home care assessment and take the first step toward peace of mind for your family.
