At Moore Care, all assessments are completed by Sheri, our Director of Care, who proudly serves families throughout the Greater Baton Rouge Area. With years of experience in senior care, Sheri takes the time to listen carefully, observe thoughtfully, and ensure no detail is overlooked.

The assessment fee is $75, and the good news is that this fee is refunded on your first invoice once services begin.

Who Should Be Present?

We encourage clients, family members, and anyone involved in care decisions to be present if possible. Having loved ones there allows everyone to ask questions, share concerns, and feel confident moving forward together.

What Happens During the Assessment?

Every assessment is tailored to the individual, but most visits include the following components:

1. Conversation and Care Goals

Sheri will begin by getting to know you, your routines, preferences, concerns, and goals. This is your opportunity to share what’s working well, what’s becoming challenging, and what kind of support would be most helpful.

2. Health and Safety Review

The assessment includes a general overview of:

  • Medications and mobility considerations
  • Fall risks and home safety concerns
  • Memory, cognitive, or behavioral changes

This helps us determine the safest and most effective approach to care.

3. Daily Living Needs

We’ll discuss how assistance may be needed with activities of daily living, such as:

  • Bathing and personal hygiene
  • Dressing and grooming
  • Meal preparation and nutrition
  • Light housekeeping
  • Transportation or companionship

The goal is to provide the right level of help while maintaining independence and dignity.

4. Personality, Preferences, and Routine

Great care goes beyond tasks. Sheri will take time to learn about personal preferences: likes, dislikes, routines, and personality traits, so we can match caregivers and services appropriately.

Creating a Personalized Care Plan

After the assessment, Sheri uses the information gathered to determine the appropriate care level and design a personalized care plan. This plan outlines recommended services, scheduling, and any special considerations to ensure care feels seamless and supportive.
